Participation Waiver & Release Agreement
I, the parent/guardian of *|firstname|* *|surname|*, understand that participation in martial arts, physical activity, games, drills, and related training activities involves inherent risks, including but not limited to falls, collisions, physical contact, strains, sprains, and other injuries.
I acknowledge that these risks exist regardless of the care, instruction, supervision, or precautions taken by North Brigade Academy Ltd.
By registering for Little Wolf Brigade, I voluntarily choose to allow my child to participate and accept responsibility for the risks associated with participation.
Creating a Positive Training Environment
I understand that Little Wolf Brigade is designed to be a fun, safe, and encouraging environment for young children to learn and grow.
I agree to support my child in:
- Listening to coaches and following class instructions.
- Treating coaches, teammates, and the training space with kindness and respect.
- Participating safely and giving their best effort.
- Communicating any discomfort, injuries, or concerns to coaches.
I understand that coaches may redirect, modify activities, or remove a child from an activity if needed to maintain a safe and positive environment for all participants.
Health & Emergency Information
I confirm that my child is physically able to participate in martial arts activities. I agree to inform North Brigade Academy Ltd. of any medical conditions, injuries, allergies, limitations, or other information that may affect safe participation.
In the event of an emergency, I authorize North Brigade Academy Ltd., its coaches, staff, or representatives to seek appropriate medical attention for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.
Release of Liability
In consideration of being permitted to participate in Little Wolf Brigade, I release and hold harmless North Brigade Academy Ltd., its owners, coaches, instructors, employees, volunteers, and representatives from any claims, demands, damages, expenses, or liability arising from participation in the program, except where such liability cannot legally be excluded.
This release applies to participation in classes, training activities, events, demonstrations, and other academy-related activities.
Photography & Media Consent
I understand that photographs, videos, and other media may be captured during Little Wolf Brigade classes, events, demonstrations, and academy activities.
I grant North Brigade Academy Ltd. permission to use photographs and videos of my child for promotional, educational, and marketing purposes, including social media, website content, advertising, and other academy communications.
Personal Belongings
I understand that participants are responsible for their personal belongings and required clothing/equipment. North Brigade Academy Ltd. is not responsible for lost, stolen, or damaged items.
Refund Policy
Registration in Little Wolf Brigade reserves a spot for your child in the Fall Adventure Session. Due to limited class sizes and preparation required for each session, registration fees are non-refundable once the session begins.
Refund requests made before the first class may be considered less any applicable processing fees.
Missed classes due to illness, vacations, scheduling conflicts, or other personal reasons are not eligible for refunds or credits.
In the event that North Brigade Academy Ltd. must cancel the session, appropriate arrangements will be made.
Acknowledgement
By registering for Little Wolf Brigade, I confirm that I have read, understood, and agree to the terms of this waiver, release agreement, media consent, and refund policy.
