Please complete details so we can welcome you to our club to try out the fantastic martial art of Brazilian Jiu Jitsu.

This form is for Adult membership (please get in touch for kids/youth membership).

It's essential that we have a record of who attends the club, and that we ask you to complete a waiver as this is a physical activity.
When you complete your first class we can discuss membership options.

We look forward to welcoming you into our community.

Please arrive at least 15 minutes early to allow time to get changed.
We have limited parking so its best to park around Pargolla or Springfield road or use one of the town car parks.
