By completing this form and the waiver, you are agreeing to use electronic documents. You also agree to be bound to all terms of this agreement.

Members will register a participant(s) / student(s) to engage in training in martial arts including self-defense and / or Tai Chi. These activities have inherent risk of injury. Participants and / or their legal guardians acknowledge this risk and agree to hold Bakersfield Elite Martial Arts (BEMA) harmless by agreeing to and signing our Waiver.

BEMA agrees to provide training to participants, including use of our facility and equipment at 1201 24th St. B200 Bakersfield, CA 93301 in exchange for a fee based on our current rates. Members will receive training for the number of days agreed upon, at the times described on our website, based on the membership type selected. Missed days during any week may be made up within the following 2 weeks.
BEMA is closed on several days throughout the year including Federal Holidays and more. Members agree that those days are excluded from this membership agreement. Days can be seen on the calendar on our website. Days are subject to change. No discounts, reimbursements, or refunds will be made based on these closures.

Members / participants agree to abide by the rules posted inside the facility and/or any additional rules provided verbally by the instructors. Failure to follow the rules may result in immediate termination of membership without reimbursement / refund of any kind.

This is a month to month agreement. Members may pause payments for documented medical reasons or as allowed by the owner, David Woods. This pause may be for one month only. If additional months are needed, member must inform BEMA in writing or in person at 1201 24th St. B200.

If the member wishes to cancel their membership, they must inform BEMA in writing 60 days in advance.
